Job summary

• Maintains essential life support certifications and trauma course requirements.

Responsibilities

• Utilizes nursing process to assess patients and develop a plan of care with the interprofessional team.
• Coordinate care plans, manage hospice caseload, and ensure quality end-of-life care.
• Collaborate with facilities and families to support patient goals and outcomes.
• Respond to crises, mentor staff, and ensure patient safety through evidence-based practice.
• Maintain positive relationships with physicians and other care team members.
