Job Summary:
The Hospice Registered Nurse (RN) provides compassionate, patient- and family-centered care to individuals with terminal illnesses. The RN coordinates and delivers physical, emotional, and spiritual support to patients in their homes, hospice facilities, or long-term care settings, ensuring comfort, dignity, and quality of life in the end-of-life journey.
Key Responsibilities:
Conduct initial and ongoing comprehensive assessments of the patient’s physical, emotional, and psychosocial status.
Develop and implement individualized hospice care plans in collaboration with the interdisciplinary team.
Administer medications and treatments for symptom and pain management according to physician orders.
Provide education and support to patients and their families about the disease process, hospice philosophy, and end-of-life care.
Maintain accurate, timely documentation in the electronic medical record (EMR) in accordance with hospice policies and regulations.
Serve as a patient advocate and coordinate care with physicians, chaplains, social workers, aides, and volunteers.
Participate in on-call rotations, after-hours visits, and crisis intervention as needed.
Ensure all care is provided in accordance with established standards of practice, agency policies, and applicable laws and regulations (including Medicare/Medicaid requirements).
Qualifications:
Licensure: Active and unrestricted Registered Nurse (RN) license in the state of practice.
Education: Graduate of an accredited school of nursing (ADN or BSN).
Experience: Minimum of 1 year of nursing experience; hospice, palliative, or oncology experience preferred.
Certifications: Current CPR/BLS certification; CHPN (Certified Hospice and Palliative Nurse) preferred or willingness to obtain.
Skills: Strong clinical judgment, excellent communication, emotional resilience, and empathy.
Technology: Proficient in electronic documentation and basic computer systems.
Working Conditions:
Frequent travel to patient homes, long-term care facilities, or assisted living residences.
May involve exposure to emotional situations, infectious diseases, and varying home environments.
Flexibility in scheduling, including nights, weekends, holidays, and on-call duties.
