Job summary

VitalCaring – Deliver Care That Brings Comfort and Dignity is seeking an experienced RN to lead compassionate end-of-life care in patients' homes and facilities. You will develop individualized, comfort-focused care plans and coordinate complex symptom management with an interdisciplinary team.

This field-based role requires strong clinical judgment, autonomy, and excellent communication to support patients and families while maintaining high-quality documentation and compliance.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care by conducting comprehensive assessments and developing comfort-focused care plans.
• Deliver skilled nursing care with emphasis on pain and symptom management.
• Serve as Care Coordinator by coordinating care across the interdisciplinary team and monitoring patient condition.
• Support Patients & Families with education, emotional support, and guidance throughout the journey.
• Drive Quality & Compliance through accurate documentation and participation in care planning meetings.
