Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing position in Monroeville, Pennsylvania.

This full-time role supports patients in Monroeville, PA and surrounding areas, with an emphasis on one-on-one patient care and care plan management. A $15,000 total bonus potential is offered, including a $10,000 first-year success bonus and a $5,000 sign-on bonus. Flexible scheduling and team-based support are provided.

Qualifications

• RN license in the state you work.
• Associate degree or higher from an accredited School of Nursing.
• Two years of RN experience, hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to meet deadlines for charting.
• Skills to self-manage time and schedule.
• Experience with tablets, mobile phones and EMR software.

Responsibilities

• Grounded in team-based care to manage and deliver patient care plans.
• Provide hospice RN services to patients in Monroeville and surrounding areas.
• Collaborate with AHN Healthcare@Home team to ensure high-quality care.
