High Plains Senior Care Hospice is seeking a full-time Registered Nurse (RN) to provide compassionate nursing care in patients' homes in Brady, Texas. The role involves coordinating care with the interdisciplinary team, patients, families, and referral sources, ensuring adherence to the Plan of Care and agency policies.

The RN will participate in IDG meetings, document services, and support ongoing caregiver education. A $2,500 sign-on bonus is offered for this Brady, TX position.

Qualifications

• RN license required (Texas) and CPR certification.
• Strong communication and interpersonal skills.
• Reliable transportation with a valid driver's license.

Responsabilités

• Promote hospice mission, goals and philosophy.
• Coordinate with IDG and patient/family to implement the plan of care.
• Provide nursing care in patients' homes or residence.
• Document services and report changes to the IDG.
• Participate in ongoing education as required.
• Perform on-call duties as assigned.
• Other duties as assigned.
