Job summary

UPMC Family Hospice and Palliative Care in Altoona and surrounding area seeks an RN to provide one-to-one patient care in home and facility settings. You will work with a multidisciplinary team to support patients and families, delivering Life Changing Medicine on a daily basis.

Qualifications

• Current licensure as a Registered Nurse in the relevant state.
• CPR certification as per AHA standards.
• BSN preferred; eligible for RN licensure in applicable states.

Responsibilities

• Coordinates care with an interdisciplinary team to ensure appropriate services.
• Provides direct patient care and documents accurately in the electronic system.
• Attends case conferences and participates in on-call and weekend coverage.
• Demonstrates life-span knowledge and adheres to policies and procedures.
