Qualifications

• Must be a Registered Nurse licensed in the state of operation.
• Nursing experience required, preferably in Hospice or Home Health.
• Sound knowledge of nursing principles and patient family teaching.

Responsibilities

• Assess, monitor, and document patient’s symptoms using required software.
• Collaborate with patients, families, and interdisciplinary teams for care plans.
• Serve as patient advocate, informing on rights and responsibilities.
• Observe and evaluate patient’s status and determine caregiver education needs.
• Supervise aides and licensed practical nurses as per guidelines.
• Participate in on-call schedule and provide backup assistance.
