Job summary

Caris Healthcare is seeking a Registered Nurse (RN) to provide compassionate hospice/home health care across the Southeast region. The role involves assessing and documenting patient status, developing individualized care plans, and collaborating with families and the care team.

Qualifications

• Must hold an RN license in the operating state and have hospice or home health experience preferred.
• Knowledge of nursing principles, procedures and patient-family teaching.
• Ability to assess, plan, implement and evaluate total patient care plans.

Responsibilities

• Assess, monitor, and document patient symptoms using required software symptoms.
• Collaborate with patient, family, partners, and interdisciplinary team to develop individualized plans of care.
• Advocate for patients, explain rights and procedures, and support families.
• Observe patient status, safety, and education needs for caregivers.
• Supervise and direct aides and licensed practical nurses according to guidelines.
• Participate in on-call rotation and provide back-up as needed.
