Job summary

Amedisys is seeking a Hospice Registered Nurse to support patients in their homes in Roanoke, VA. This on-site position offers flexible hours with a per diem option and mileage reimbursement, plus a clear path for growth within one of the nation\'s leading home health and hospice providers.

Responsibilities include initial assessments, care planning, progress communication to the care team, and family education.

Qualifications

• Current RN license, state-specific for the assignment.
• Current CPR certification.
• Valid driver\'s license with reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the care team.
• Communicates patient progress to the clinical manager, physician, and team.
• Maintains patient care progress records accurately and timely.
• Records pain and symptom management changes/outcomes.
• Educates families about patient condition and care needs.
• Makes referrals to hospice disciplines as needed.
• Performs on-call responsibilities as assigned.
• Performs other duties as assigned.
