Job summary

Northwell Health seeks a dedicated Registered Nurse to provide age-appropriate care using the nursing process and collaborate with physicians and the health team in a Hospice setting.

The role emphasizes leadership, clinical judgment, and coordination on the patient care unit, including supervising aides and coordinating plans of care across disciplines. Field position with on-site requirements in New York.

Qualifications

• Graduate from an accredited School of Nursing; a Bachelor's Degree in Nursing is preferred.
• Current NY State license to practice as a Registered Professional Nurse is required, with additional certifications as needed.
• 2+ years nursing experience in med/surge, hospice, or home care; acute care experience strongly preferred.

Responsibilities

• Conducts pre-admission screening and initial nursing health assessment; develops plan of care and goals.
• Provides nursing care including supervision of diet, medication, activity levels and home safety.
• Evaluates nursing care including therapy services and coordination with sponsoring agencies.
• Trains and evaluates home health aides and personal care workers in the patient's home.
• Determines need for medical equipment and supplies and monitors equipment condition.
• Recommends therapies via the Hospice IDG and coordinates physician orders as needed.
• Participates in on-call coverage and maintains Plan of Care during inpatient transitions.
• Engages in ongoing Quality Improvement activities.
• Recognizes physical demands of the role including lifting and mobility tasks.
