About The Role

Ennoble Care is hiring for a Full-time, Hospice Registered Nurse Case Manager who resides within or in close proximity to Somerset County, NJ. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician, and the interdisciplinary group in providing skilled nursing assessments, planning, and care to maximize the comfort and health of both patients and their loved ones/caregivers.

Responsibilities

• Assign patient caseloads and conduct assessments, planning, implementing, and evaluating nursing care.
• Coordinate care with attending physicians, hospice staff, and other agencies.
• Maintain communication and documentation regarding patient care.
• Participate in team meetings and provide support to caregivers.
• Supervise and document care provided by Nursing Assistants.
• Support patients at the time of death and during bereavement.
• Participate in training and community programs.
• Stay informed about policies and regulations governing hospice care.

About You

• Current RN license in NJ.
• At least two years of nursing experience, preferably including hospice care.
• Strong assessment and communication skills.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation.
