Job summary

Mission Healthcare is seeking a Hospice RN Case Manager to support patients at home. The role manages assessments, plans and coordinates care, and ensures timely revisions to the care plan as patient needs change.

The RNCM collaborates with families, administers medications, and provides education to support the plan of care. A valid RN license, BLS, and driver’s license required, with preference for home health experience.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), with hands-on CPR certification.
• Valid driver's license and auto insurance.
• Bachelor’s degree in Nursing, and 1 year of home health experience preferred.

Responsibilities

• Performs initial patient and family assessment to determine home care needs and documents the physical assessment and medical history.
• Initiates the plan of care and revises it as patient status changes.
• Regularly re-evaluates patient needs and nursing diagnoses.
• Uses assessment data to determine nursing diagnoses and develops an aligned care plan with goals.
• Performs nursing actions, administers prescribed medications and treatments.
• Counsels patient and family on plan of care and related needs.
• Provides health care instructions to patient as appropriate.
• Identifies discharge planning needs and implements before discharge.

Pay range (depending on experience): $55.00-$60.00 + Sign on Bonus $2,500

Schedule/Shift

Full Time, Monday-Frifday, 8:00am-5:00pm

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
