Hartford HealthCare at Home is seeking a Weekday Primary RN Case Manager to support patients and families at end‑of‑life stages. You will conduct comprehensive assessments to establish goals for symptom management and collaborate with an interdisciplinary team to ensure holistic care.

Applicants must hold a CT RN license, have at least one year of RN experience (homecare/hospice preferred), and possess a valid driver’s license with reliable transportation.

Qualificações

• Registered Nurse with Valid CT RN license.
• One year of RN experience; Homecare or Hospice experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsabilidades

• Support patients and families through end‑of‑life journey.
• Perform a comprehensive assessment to establish goals for symptom management and end‑of‑life care with effective case management strategies.
• Be a member of an active interdisciplinary team that addresses patients’ needs holistically.
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
• Utilize state‑of‑the‑art technology to document skilled services provided.
