Emblem Hospice in Arizona is seeking a compassionate Registered Nurse to join our field-based care team. As a Hospice RN, you will deliver in-home and community-based care, guide families through care decisions, and provide comfort, dignity, and support during life’s tender moments.

This full-time role requires a valid AZ RN license, at least two years in nursing (hospice/home health preferred), and a reliable vehicle.

Competenze

• Active RN license in Arizona.
• Minimum 2 years nursing experience; hospice or home health preferred.
• Valid driver's license and reliable vehicle.
• Homecare Homebase experience preferred.

Mansioni

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
