Job summary

Del Corazon Hospice is seeking a compassionate Registered Nurse (RN) to provide hospice visits across Los Alamos, White Rock and surrounding communities. The home office is in Santa Fe and travel to Santa Fe for IDT and meetings is expected.

The RN will manage symptom control, administer medications per plan, educate families, and collaborate with the interdisciplinary team, while maintaining thorough documentation and patient-centered care.

Qualifications

• Active RN license in New Mexico (required).
• At least 1 year of nursing experience (hospice/home health strongly preferred).
• Strong assessment, communication, and independent decision-making skills.
• Ability to work autonomously in the field with excellent time management.
• Valid driver’s license and reliable vehicle.
• CPR certification preferred.

Responsibilities

• Case manage hospice patients in Los Alamos and surrounding communities.
• Participate in nurse on-call rotation; paid on-call stipend and after-hours pay.
• Conduct patient assessments and manage pain/symptom control.
• Administer medications and treatments per physician plan of care.
• Educate and support patients, caregivers, and families.
• Collaborate with the interdisciplinary team (IDT).
• Complete timely documentation in the EMR.
• Advocate for comfort-focused, patient-centered care.

Compensation: $88,000 to $100,000 based on relevant experience, clinical skills, independence, documentation quality, and overall fit for the role. Mileage reimbursement, paid on-call stipend, and additional after-hours visit pay provided.

Del Corazon Hospice is hiring a compassionate and dedicated Registered Nurse (RN) to provide hospice nursing visits to patients throughout Los Alamos, White Rock and surrounding communities. Home office is in Santa Fe and will need to travel to Santa Fe for IDT and meetings.

Position Summary

The Hospice RN provides skilled nursing care, symptom management, and compassionate support to hospice patients wherever they call home. This role partners closely with the interdisciplinary team to ensure comfort, dignity, and excellent end-of-life care.

Key Responsibilities

• Case Manage hospice patients along with needed visits in Los Alamos and surrounding communities
• Participates in nurse on-call rotation for weeknight coverage, Monday through Thursday. On-Call is typically one night per week, but may vary depending on the number of nurses in the rotation. Paid on-call stipend and additional after-hours visit pay are provided.
• Conduct patient assessments and manage pain/symptom control
• Administer medications and treatments per physician plan of care
• Educate and support patients, caregivers, and families through the hospice journey
• Collaborate with the hospice interdisciplinary team (IDT) including social work, chaplain, aides, and medical director
• Complete timely and accurate documentation in the electronic medical record
• Serve as an advocate for comfort-focused, patient-centered care
