Unified Care Hospice – Dallas is seeking a compassionate and dedicated Registered Nurse Case Manager to join our mission-driven team. At Unified Care Hospice, we believe hospice care is a partnership—one that provides holistic, patient-centered care that honors each individual’s journey.

As an RN Case Manager, you will play a pivotal role in ensuring comfort, dignity, and quality of life for our patients and their families. We offer competitive pay, flexible scheduling, and comprehensive benefits.

  • Conduct comprehensive patient assessments and create individualized care plans.
  • Manage pain and symptoms with compassion and clinical expertise.
  • Collaborate with physicians, hospice aides, social workers, chaplains, and volunteers.
  • Educate and emotionally support patients and families during the end-of-life journey.
  • Accurately document all visits and interventions in the EMR in compliance with CMS and ACHC standards.
  • Participate in weekly interdisciplinary team meetings (IDG).
  • Serve as a resource for patients, families, and referral sources.
  • Promote Unified Care Hospice’s values by referring aligned professionals.
  • Active RN license in the State of Texas (required).
  • Minimum 1 year of hospice, palliative, or home health experience (preferred).
  • Knowledge of ACHC standards, CMS Conditions of Participation, and Texas hospice regulations.
  • Strong communication, assessment, and time management skills.
  • Proficiency in EMR systems (WellSky or Axxess preferred).
  • Current CPR certification.
  • Compliant with agency COVID-19 policies, including documentation and education per state law (vaccination not required).
  • Full-time, Monday–Friday.
  • Rotating on-call and weekend availability.
  • Additional hours may be available on holidays or as needed.
