Job summary

Your Health Organization in the Gainesville area seeks a Registered Nurse to join our growing primary care family as a Nurse Case Manager. The RN will travel to patients’ homes and facilities, coordinating with providers and the care team to manage comprehensive care plans.

Responsibilities include reviewing records, enrolling patients in Care Management, and documenting activities while ensuring compliant, quality care. A valid RN license and ability to travel are required.

Qualifications

• Must be a Registered Nurse with license in good standing.
• Minimum of 3 years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to lead and work as part of a team.
• Must be able to travel between offices and facilities.

Responsibilities

• Facilitate receiving medical records from patient providers and specialists.
• Review medical records and enroll patients in Care Management.
• Initiate a Care Management Plan of Care if eligible.
• Coordinate telehealth visits and visits to patients' homes or facilities.
• Document Care Management activities and maintain up-to-date care plans.
• Coordinate with patient health care team and support services.
