Job summary
Hospice of Chattanooga is seeking a full-time Hospice Registered Nurse Case Manager to join our compassionate team. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey in Grundy County.
The role offers Monday–Friday, 8:00 am–4:30 pm with a supportive environment, comprehensive benefits, and opportunities for growth within a mission-driven organization.
Qualifications
- Hold current unencumbered license as a Registered Nurse.
- Minimum one year experience as an RN in a medical-surgical/acute care setting.
- Must be computer proficient with EMR.
- Current CPR certification as required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Empower Through Engagement: respond to clinical referral information with care and expertise.
- Collaborative Care Champion: assess eligibility and suitability for hospice services.
- Nurturing Direct Care: coordinate care while maintaining patient comfort and quality of life.
- Nursing with Heart: deliver top‑notch care rooted in nursing principles.
- Educator and Advocate: educate patients and families for personalized care plans.
- Call of Compassion: participate in on-call duties as part of the team.
