Job summary

Rainbow Community Care in Wisconsin is seeking a Registered Nurse Case Manager for home hospice to plan, coordinate, and deliver patient/family care and to implement physician orders with the primary care physician. This full-time hybrid role requires driving to patient homes and facilities and collaborating with an interdisciplinary team.

You will educate families on care skills, medications, treatments, and signs of dying and grief, participate in biweekly meetings, and update nursing plans as

Qualifications

• Wisconsin RN license required and must be maintained.
• Valid driver's license and auto insurance required.
• Hospice, Palliative Care or Home Health experience preferred.
• Ability to drive to patient homes and care facilities.

Responsibilities

• Develops and supervises planning, coordination, and delivery of patient/family care.
• Implements physician orders and communicates with primary care doctor.
• Educates family on care skills, meds, treatments, and signs of dying/grief.
• Participates in biweekly interdisciplinary team meetings.
• Monitors patient changes and updates care plans accordingly.
• Documents medications, care, and diagnoses in chart; ensures timely charts.
