About the Role:

Ennoble Care is hiring a Full-time, Hospice Registered Nurse Case Manager for Hudson County, NJ. The RN Case Manager works under the general direction of the Director of Nursing, the attending physician, and the interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize patient comfort and health, along with supporting their loved ones and caregivers.

The responsibilities include:

  1. Managing a patient caseload assigned by the Patient Care Coordinator or designee.
  2. Assessing, planning, implementing, and evaluating patient care.
  3. Communicating with physicians, hospice staff, and other agencies to coordinate care.
  4. Maintaining communication with the attending physician regarding patient status.
  5. Collaborating with team members for the patient's care plan.
  6. Documenting care accurately in electronic medical records.
  7. Participating in interdisciplinary meetings and providing input.
  8. Coordinating physical care and teaching caregivers or providing direct care as needed.
  9. Reviewing caseload issues with hospice nursing staff and sharing feedback.
  10. Attending care conferences and facilitating discussions.
  11. Supervising and documenting the NAC biweekly or as assigned.
  12. Reporting issues to the Patient Care Coordinator.
  13. Responding to patient and family needs during on-call hours.
  14. Providing support during end-of-life and bereavement periods.
  15. Participating in orientation and training programs.
  16. Engaging in community programs to promote professional growth.
  17. Familiarity with agency policies and regulatory requirements.
  18. Performing other relevant duties as assigned.

Qualifications:

  • Graduate of an accredited School of Nursing.
  • Licensed in NJ as a Registered Nurse.
  • Minimum of one year RN experience; hospice experience preferred.
  • Excellent assessment and problem-solving skills.
  • Strong organizational and time management skills, with knowledge of case management.
  • Valid driver’s license with insured vehicle.
  • Effective verbal and written communication skills.
  • Fluent in English and Spanish.

Pay Rate: Starting at $100,000/yr, with potential for higher depending on experience.
