AHN Healthcare@Home is seeking an experienced Registered Nurse (RN) for an in-home hospice role in the Coraopolis, PA area. The role includes delivering compassionate hospice care, performing assessments, and supporting patients and families in end-of-life care.

The position requires an active RN license in the state, at least two years of RN experience (hospice preferred), and a valid driver's license with ~20% travel to patient homes. A $10,000 first-year success bonus is offered.

Qualifications

• Active Registered Nurse license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Must have a valid driver's license and ability to drive ~20% of work time.

Responsibilities

• Deliver patient-centered hospice care in patients' homes.
• Perform assessments, symptom management, and end-of-life care planning.
• Educate and support patients and families throughout the journey.
• Document thoroughly in EMR and coordinate with the interdisciplinary team.
