Job summary

VNA Care is seeking a dedicated Hospice Registered Nurse Case Manager for a Mon–Fri, 8:00 AM–4:30 PM schedule serving Marlboro, Framingham, Holliston and Franklin areas. You will assess patients, develop and implement care plans, and support families through end-of-life care with compassion and professionalism.

Compensation is based on experience at $93,600–$97,760 per year. Benefits include medical, dental and vision plans, EAP, 401K, life and disability insurance, and flexible spending

Qualifications

• Minimum of one year in an acute care setting or comparable work experience.
• Minimum of one year oncology and home care experience preferred.
• Minimum of one year Hospice experience preferred.

Responsibilities

• Assess patients' needs and develop care plans.
• Implement care plans and coordinate services for end-of-life care.
• Support patients and families to ensure comfort and quality of life.
