Job summary

Andwell Health Partners, a trusted hospice provider, seeks a Hospice Registered Nurse to deliver skilled care in patients' homes across Presque Isle, ME and surrounding towns. You will assess needs, create care plans, manage pain, and educate families with a focus on comfort and dignity.

Qualifications

• Current Maine RN license.
• Minimum 1 year of recent clinical nursing experience.
• Knowledge of pain management and symptom control.
• Strong assessment, communication, and critical thinking skills.
• Compassion, empathy, and patient-centered care.
• Reliable transportation and current auto insurance.
• Hospice nursing experience is a plus.

Responsibilities

• Deliver skilled nursing visits in patients' homes per physician plans of care.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Develop individualized care plans with measurable outcomes with patients/families.
• Manage pain and symptoms to maximize comfort.
• Educate patients and families to support informed decisions.
• Advocate for patient rights and goals of care.
• Collaborate with an interdisciplinary hospice team.
• Provide reassurance and compassionate support.
