Friendship Village, a premier Life Plan Community in St. Louis, is seeking a qualified RN Case Manager to join our compassionate team serving active-minded senior adults.

The role focuses on end-of-life care with a team-oriented approach to ensure comfort and quality of life for patients. The Medical Social Worker provides medical social services to patients and families under the Plan of Care, within a supportive, faith-based community that values mission and compassionate care.

Responsibilities

• The RN Case Manager cares for patients with terminal illnesses or other diagnoses, indicating they have fewer than six months to live.
• The RN Case Manager will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life.
• The Medical Social Worker provides medical social services to the patient and/or family according to the Plan of Care.
