Job description

The Hospice RN, Case Manager plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The RN, Case Manager builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Responsibilities
  • Completes an initial assessment of patient and family to determine hospice care needs.
  • Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs.
  • Initiates and oversees the plan of care and makes necessary revisions as patient status and needs change. Communicates updates as necessary.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Acts as member of Interdisciplinary Team and participates in IDT meetings and case conferences.
Education and Work Experience
  • Required Degree: Associate degree, Nursing
  • Preferred Degree: Bachelor’s Degree, Nursing
  • Preferred Certificate(s): RN License and CPR, if required by state
  • Experience: 1+ Years of hospice care experience preferred
Knowledge, Skills, and Ability
  • Ability to communicate effectively with residents and healthcare professionals.
  • Knowledge of cultural, social, and economic factors that influence patient care and healthcare access.
  • Ability to report and document financial information as needed.
  • Knowledge of privacy regulations and maintaining patient confidentiality
Physical Requirements
  • Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
  • Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
  • Ability to operate various equipment base on department.
  • Ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms.
  • Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.
  • Clarity of Vision: § For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers. § For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well. § For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation Pay Range: $38.74 - $54.51 per hour Reasonable Pay Estimate A reasonable estimate of the pay range for this position is $38.74 - $54.51 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
