Qualifications

• RN license (active in Texas) and clinical experience in hospice, palliative, geriatric, or home health preferred.
• Calm under pressure with strong listening skills and compassionate bedside manner.
• Independent, collaborative, and integrous with a patient- and family-centered approach.

Responsibilities

• Deliver skilled nursing care to patients in their home environments.
• Collaborate with interdisciplinary teams to coordinate patient care and communicate with providers.
• Provide clinical, emotional and educational support to families and caregivers.
• Advocate for patients by educating about conditions, options and resources.
• Work with a team dedicated to dignified, quality of life care.
