Qualifications

• RN license active in state of practice.
• Graduate from an accredited nursing program (Diploma/Associate/Bachelor).
• Two years RN experience preferred in healthcare/clinical setting.

Responsibilities

• Conduct initial and ongoing assessments of patients with life-limiting illnesses and their needs.
• Implement the individualized care plan and revise as necessary.
• Collaborate with physicians, caregivers, and other healthcare professionals.
