Job summary

JOL Healthcare is seeking a Hospice RN Case Manager to coordinate and deliver high-quality hospice care in patients’ homes and hospice facilities. You will lead comprehensive assessments, develop care plans, and work with the interdisciplinary team to ensure compassionate, compliant care.

The role requires strong clinical skills, excellent communication, and ability to manage a caseload with minimal supervision while traveling to patient locations.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.

Responsibilities

• Coordinate and provide hospice nursing care to patients in homes and hospice facilities.
• Develop and implement individualized care plans.
• Document all patient interactions in the EMR system.
• Collaborate with physicians, social workers, chaplains and the interdisciplinary team.
