Job summary

Qualifications

• Spanish-speaking candidates preferred.
• Current RN licensure (in good standing) in the state(s) of practice.

Responsibilities

• Provide patient care visiting patients in homes and facilities.
• Collaborate with interdisciplinary team (nurses, social workers, chaplains, CNA leadership).
• Contribute to psychosocial assessments and care planning.
• Travel across the service area to visiting patients and documented visits.
