Position Overview

This role involves clinical management of end-of-life patients, working with an interdisciplinary team to develop and implement personalized care plans. The position is eligible for an annual performance bonus.

Responsibilities

• Manage a caseload of patients and families, including assessments, care planning, and evaluation.
• Assess patient needs for admission and re-evaluate clinical status at least every 14 days.
• Coordinate and document care plans with the interdisciplinary team, delivering skilled nursing services using evidence-based practices.
• Document all clinical visits and care activities within 24 hours.

Qualifications

• Preferred Bachelor's degree.
• State-issued RN license required.
• Minimum two years' experience, preferably in hospice or discharge planning.
• Ability to communicate compassionately about end-of-life issues and respond calmly under stress.
• Proficiency in Word, Excel, and documentation software.
