Haven Hospice in Lake City, FL is seeking a compassionate Registered Nurse Case Manager to provide expert, patient-centered hospice care. This full-time role supports individuals and families during a difficult time, coordinating plans and services to honor patient goals and comfort at home.

You will assess needs, develop care plans, administer treatments, educate families, and collaborate with an interdisciplinary team. The schedule is Monday through Friday, 8 a.m.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Possess and maintain current CPR certification where required.

Responsabilités

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
