Haven Hospice in Florida is seeking a Registered Nurse Case Manager to deliver patient-centered hospice care across Deland, Daytona Beach, and Palm Coast. You will assess needs, develop care plans, administer treatments, and support families while collaborating with a diverse team.

Qualifications

• Graduate of an accredited nursing program with active state licensure.
• Minimum one year nursing experience; three+ years preferred.
• Hospice experience or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver's license and insured vehicle.

Responsabilités

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligning with patient wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
