Qualifications

• Graduation from an accredited school of nursing.
• 1-2 years of recent acute care experience in an institutional setting (required). 1-2 years of recent home care experience (preferred).
• Valid driver's license and insured automobile in good working order.

Responsibilities

• Performs initial, comprehensive assessments of patients and families to determine hospice needs.
• Plans and directs patient care using the nursing process and develops a plan of care.
• Initiates and revises care plans as patient status changes; coordinates with the physician.
