Job summary

BAYADA Home Health Care in Bucks County, PA is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients at home across Montgomery and Bucks Counties. You will join a multidisciplinary team and provide palliative care in patients' homes.

As part of Jefferson Health at Home by BAYADA Hospice, you will perform home visits, administer medications, wound care, and comfort care; monitor conditions and document orders; and participate in a team with flexible daytime

Qualifications

• Active PA RN license required.
• Hospice experience preferred.
• Home healthcare experience welcomed, but not required.
• Ability to travel to assigned cases.
• Compassionate, reliable clinician with strong communication.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform administration of medication, wound care, treatments, and comfort care.
• Monitor clients' conditions; report changes to Clinical or Client Services Manager.
• Follow doctors' orders and properly document their orders.
• Occasional on call after hours and/or weekends.

We are currently recruiting for a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients in small territories throughout Montgomery County and Bucks County, PA.

What you’ll do:

As a Jefferson Health at Home by BAYADA Hospice Nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home. You will:

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Occasional on call after hours and/or weekends

What you should have:

• Active and current license to practice as a professional registered nurse (RN) in the state of PA.
• One year of hospice experience strongly preferred.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.
