Job summary

BAYADA Home Health Care is recruiting for a compassionate Hospice Registered Nurse (RN) Case Manager to serve patients across Montgomery and Bucks Counties, PA. You will join a multidisciplinary team delivering high-quality, compassionate care in patients' homes.

As an RN with hospice experience, you will perform medications, wound care, and routine treatments, while documenting care and coordinating with physicians. Flexible schedules and on-call opportunities are available.

Qualifications

• Must hold an active PA RN license.
• Hospice experience strongly preferred.
• Ability to travel to cases as assigned.
• Excellent communication and interpersonal skills.

Responsibilities

• Make home visits to patients in designated territories.
• Administer medications, perform wound care, treatments and procedures.
• Monitor patient conditions and report changes to supervisors.
• Follow doctors' orders and document care accurately.
• Occasional on-call after hours and weekends.

We are currently recruiting for a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients throughout in small territories throughout Montgomery County and Bucks County, PA.

What you’ll do:

As a Jefferson Health at Home by BAYADA Hospice Nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home. You will:

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Occasional on call after hours and/or weekends

What you should have:

• Active and current license to practice as a professional registered nurse (RN) in the state of PA.
• One year of hospice experience strongly preferred.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.
