What You’ll Do:

• Manage a caseload of hospice patients, providing hands-on nursing care and end-of-life support

• Coordinate with families, providers, CNAs, and the interdisciplinary team to ensure quality, compassionate care

• Maintain accurate and timely documentation in EMR

• Support and educate families through the hospice journey
