Position Summary: Hospice RN Case Manager

The Hospice RN Case Manager provides professional nursing care to hospice clients, in collaboration with physicians and the interdisciplinary team. Responsibilities include client assessment, care planning, symptom management, and supervision of care outcomes. The RN collaborates with family/caregivers, anticipates patient needs, and supports hospice team members.

Key responsibilities:

• Manage a caseload of hospice patients
• Participate in weekly interdisciplinary team meetings
• Coordinate with primary care physicians and hospice medical directors

Minimum Qualifications

• Graduate of an approved nursing program
• Registered Nurse licensure in Pennsylvania
• At least 2 years of nursing experience in acute care, home care, or ER
• Home care experience and EPIC system experience preferred
