Job summary

Cadre Hospice in Seven Hills, OH is seeking an RN Case Manager to serve as the primary clinical advocate for patients, coordinating personalized end-of-life care with the interdisciplinary team. As an RN Case Manager, you'll serve as the primary clinical advocate for your patients, building meaningful relationships while coordinating personalized care that reflects each patient's goals, wishes, and unique needs. Working alongside physicians, social workers, chaplains, aides, volunteers, and other members of our care team, you'll help ensure every patient receives exceptional care with compassion, empathy, and respect.

What You'll Do Deliver Compassionate Clinical Care
  • Complete comprehensive nursing assessments to evaluate patients' physical, emotional, and psychosocial needs.
  • Develop and manage individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
  • Monitor patient progress and adjust care plans as needs evolve to maximize comfort and quality of life.
  • Serve as the primary clinical resource for patients and families throughout their hospice journey.
Manage Symptoms & Promote Comfort
  • Assess and manage pain and other end-of-life symptoms using evidence-based clinical practices.
  • Safely administer medications while monitoring effectiveness and potential side effects.
  • Educate patients and caregivers on medications, symptom management, and disease progression.
  • Proactively identify changes in condition and implement appropriate interventions.
Support Patients & Families
  • Build trusting relationships through compassionate communication and active listening.
  • Provide emotional support, education, and guidance during difficult conversations surrounding illness, grief, and end-of-life planning.
  • Help patients and loved ones navigate goals of care while honoring individual values, beliefs, and preferences.
  • Empower caregivers with the education and resources they need to confidently care for their loved one.
Collaborate as Part of an Interdisciplinary Team
  • Coordinate care across nursing, social work, chaplaincy, hospice aides, volunteers, physicians, and community partners.
  • Participate in regular interdisciplinary team meetings to ensure seamless, holistic patient care.
  • Coordinate transitions of care and connect patients with additional healthcare or community resources when appropriate.
  • Participate in on-call responsibilities in accordance with Cadre Hospice's On-Call Policy.
Maintain Clinical Excellence
  • Complete timely, accurate documentation within the electronic medical record (EMR/EHR).
  • Maintain compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and organizational policies.
  • Participate in quality improvement initiatives that support exceptional patient outcomes and continuous organizational growth.

Required Qualifications
  • Current, unrestricted Registered Nurse (RN) license in the state of practice.
  • Minimum of one (1) year of nursing experience.
  • Valid driver's license, reliable transportation, and the ability to travel throughout the assigned service area.
Preferred Qualifications
  • Previous hospice, palliative care, home health, oncology, or case management experience.
  • Strong clinical assessment and care planning skills.
  • Experience educating patients and caregivers.
  • Proficiency with electronic health record (EHR/EMR) documentation.
  • Excellent communication, collaboration, and relationship-building skills.
  • A compassionate, patient-first mindset and a commitment to delivering dignified, person-centered end-of-life care.
