Job summary

Cadre Hospice is seeking a compassionate Hospice Care Assistant (CNA/STNA) to support patients at home, helping with daily activities and comfort throughout the end-of-life journey.

You will collaborate with our clinical team led by the Director of Clinical Services to ensure the highest level of dignity, safety, and personalized care for patients and their families in the Wadsworth area.

Qualifications

  • Current Ohio CNA/STNA certification is required.
  • Minimum one year as a CNA/STNA in hospice, palliative care, or similar healthcare setting.
  • Valid driver’s license with reliable transportation and insurance.
  • Ability to travel within the service area and strong communication skills.

Responsibilities

  • Assist patients with activities of daily living (bathing, grooming, dressing, toileting).
  • Provide comfort measures and support per individualized plan of care.
  • Observe changes and report to supervising nurse promptly.
  • Maintain a clean, safe, and respectful home care environment.
  • Collaborate with RN case manager and hospice team.

What You'll Do

As a Hospice Care Assistant (CNA/STNA), you'll play an important role in helping patients feel comfortable, cared for, and respected throughout their hospice journey. You'll provide hands-on personal care and compassionate support to patients and their families while working closely with our clinical team.

Reporting to our Director of Clinical Services, you'll help ensure patients receive the highest level of comfort, dignity, and respect during their final stages of life. Your presence, compassion, and attention to detail can make a meaningful difference in the lives of patients and the families who love them.

Provide Compassionate Patient Care
  • Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene.
  • Provide assistance with positioning, transfers, ambulation, and mobility needs.
  • Offer companionship and emotional support to patients and families.
  • Respect each patient's dignity, privacy, preferences, and individual routines.
  • Encourage independence and provide care in a way that promotes comfort and confidence.
Promote Comfort & Quality of Life
  • Provide comfort measures according to the patient's individualized plan of care.
  • Observe patients for changes in condition, behavior, pain, or comfort level and promptly communicate concerns to the supervising nurse.
  • Assist with creating a peaceful, safe, and comfortable environment.
  • Provide compassionate presence and reassurance to patients and loved ones throughout the end-of-life journey.
Communicate & Collaborate
  • Work closely with the RN case manager and interdisciplinary hospice team to support individualized patient care.
  • Communicate changes in patient condition or needs to the appropriate member of the clinical team.
  • Build trusting relationships with patients, families, caregivers, and members of the hospice team.
  • Follow the patient’s plan of care and perform duties within the scope of your certification and role.
Support a Safe & Comfortable Environment
  • Assist with maintaining a clean, organized, and safe patient environment.
  • Change linens and assist with basic household tasks related to patient care as appropriate.
  • Follow infection prevention, safety, and patient privacy standards.
  • Complete accurate and timely documentation of care provided.

Required Qualifications
  • Current CNA/STNA certification in the state of Ohio.
  • Minimum of one year of experience working as a CNA/STNA in hospice, palliative care, or a similar healthcare setting.
  • Valid driver's license and reliable transportation.
  • Clean driving record and current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Strong communication, teamwork, and interpersonal skills.
