Job summary

Hospice RN Case Manager opportunity in the Dallas area south of Downtown Dallas, covering Cedar Hill, DeSoto, Red Oak, and Duncanville. This full-time role coordinates patient care, assesses needs, and develops individualized plans of care with the interdisciplinary team to support patients and families.

You will perform head-to-toe assessments, monitor symptoms, manage medications, educate families, and ensure timely documentation.

Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver’s license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience preferred
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized plans of care with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications

Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.
