Job summary

Assured Hospice, part of the LHC Group within the Optum family, seeks an RN for home hospice care in Centralia, WA. You will perform initial nursing evaluations and coordinate patient care within 48 hours of referral.

You will assess patient and family needs, develop the plan of care, document care, and collaborate with physicians and the IDG. A valid RN license and CPR certification are required; driving is required.

Qualifications

  • Current and unrestricted RN licensure in the state of practice.
  • Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
  • 1+ year of clinical experience.
  • Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

  • Performs an initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
  • Identifies patient and family physical, psychosocial, and environmental needs; re-assesses at least every 15 days.
  • Initiates and coordinates the plan of care.
  • Documents problems, goals, interventions, and responses to care.
  • Collaborates with patient/family, attending physician and IDG to provide daily care.
  • Instructs and supervises patient/family in self-care techniques when appropriate.
  • Maintains accurate clinical notes on the patient’s condition.
  • Ability to work in diverse home situations regardless of demographics.

Primary Responsibilities
  • Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
  • Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
  • Initiates and coordinates the plan of care
  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care
  • Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
  • Instructs and supervises the patient/family in self-care techniques when appropriate
  • Maintains accurate and relevant clinical notes regarding the patient's condition
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Required Qualifications

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $79,600 to $119,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
