Job summary

The Elizabeth Hospice in San Diego County seeks an experienced Nurse Practitioner to provide hospice and palliative care across all ages. You will assess and adjust care plans in collaboration with the IDT, consultant with medical leadership, and coordinate with families and community physicians. On-call coverage and travel are required.

We offer a Monday–Friday, 8:00 a.m.–5:00 p.m. schedule, competitive salary ($100,000–$130,000 DOE), benefits, and 401(k).

Qualifications

  • Must hold CA RN license.
  • Must hold CA NP license.
  • DEA certificate required.
  • NPI number or to be obtained within 2 months of hire.
  • MSN with NP program.
  • National certification (AANP/ANCC) within 2 months.
  • CPR/BLS card.
  • ACHPN preferred within 1 year of hire.
  • Minimum 3 years hospice/palliative care experience.
  • Computer skills and basic office equipment.

Responsibilities

  • Visit patients promptly after admission and as needed.
  • Document in electronic records per guidelines.
  • Coordinate care with IDT and community physicians.
  • Seek medical direction for plans outside standardized practice.
  • Make clinical judgments based on the whole patient picture.
  • Maintain hospice policies and regulatory standards.
  • Orient/train assigned staff and educate team.
  • Provide some 24/7 coverage and rotate weekends as needed.
  • Attend medical staff and NP meetings as directed.
  • Counsel patients and families on care, risks, and support.
  • Maintain comprehensive patient medical history updates.
  • Build relationships with staff and referral sources.

E. San Diego County Nurse Practitioner

The Elizabeth Hospice Nurse Practitioner provides quality hospice and palliative care to assigned patients across the life span (perinatal, pediatrics, and adults) while consulting with the team physician, associate medical director and chief medical officer (CMO). Consults, assesses, and evaluates the plan of care while collaborating with the IDT (interdisciplinary team), patient, family, and community physicians. Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice.

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.

Salary: $100,000-$130,000; Subject to experience and qualifications.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m.

On Call Requirement: Two (2) on-call shifts per month.

Travel: Required for patient visits.

Requirements
  • 1. Responsible for visiting patients within a short period of time following admission and as medically necessary afterwards.
  • 2. Ensure documentation is entered into the electronic record within expected guidelines. 3.Coordinates care with the interdisciplinary Team (IDT).
  • 3. Communicates all changes to IDT and community physician(s).
  • 4. Seeks medical direction for assigned patients as required including patients seen in the hospital.
  • 5. Utilizes critical thinking to interpret multiple, sometimes conflicting patient data.
  • 6. Makes clinical judgments based upon a grasp of the \"whole picture\" for patient diagnoses managing changes of condition with appropriate levels of care.
  • 7. Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
  • 8. Orients, instructs, and trains assigned personnel.
  • 9. Works with Education Program Manager to educate staff regarding patient care.
  • 10. When necessary, share in providing 24 hours, seven days a week coverage to patients and families. 12.Available rotating weekends to make patient visits as needed.
  • 11. Attends medical staff meetings, nurse practitioner meetings and other meetings as directed by the CMO.
  • 14.Consults the CMO when a plan of care is outside of standardized practice and protocols, and specialty consults are needed to manage patients care.
  • 15. Counsels patients, families, and/or caregivers as to preventative care, medical problems psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.
  • 16. Documents comprehensive medical history in patient's electronic record. Identify pertinent patient information, progress, medications, and diagnostic studies as required. Keeps the interdisciplinary team updated on changes made to patients plan of care (per agency documentation standards).
  • 17. Establishes, builds, and nurtures relationships with staff and community referral sources to facilitate program growth. Participates in community events as requested by the marketing team.
  • 18. Adhering to the E-way standards is a requirement in performing all job duties.
Qualifications
  • 1. Must be licensed as a Registered Nurse by the State of California.
  • 2. Must be licensed as a Nurse Practitioner by the State of California.
  • 3. Must have Drug Enforcement Administration (DEA) Certificate.
  • 4. Must have valid National Provider Identification (NPI) Number or must be obtained within two (2) months of hire date.
  • 5. Must have successfully completed a Nurse Practitioner (NP) Program from a recognized university and obtained a Master of Science Degree in Nursing (MSN). (Starting in year 2000, NP Program is included in the MSN).
  • 6. National Certification required or must be obtained within two (2) months of hire date: American Association Nurse Practitioner (AANP) or American Nurses Credentialing Center (ANCC).
  • 7. CPR/BLS Card.
  • 8. Advanced Certification as Hospice and Palliative Care Nurse (ACHPN) preferred but required within one year of hire date.
  • 9. Requires a minimum of three (3) years nursing experience in a hospice or palliative care setting.
  • 10. Computer skills (word processing) use of adding machine, calculator, fax, photocopier.
  • 12. Current California driver's license and proof of insurance required.
