AZ Hospice Care is seeking a Hospice RN Case Manager to join our team. You will assess, diagnose, and plan care for assigned patients, documenting progression and outcomes to ensure high-quality hospice services.

The role requires an Arizona RN license, at least 2 years of hospice experience, and the ability to work effectively in a fast-paced environment while coordinating with physicians and other healthcare professionals to support patients and families.

Qualifications

• Must have nursing or related medical field experience.
• Ability to use medical software and equipment.
• Strong interpersonal skills to build rapport with patients.
• Excellent problem-solving and critical thinking abilities.
• Ability to thrive in a fast-paced environment.
• Minimum of 2 years hospice experience.
• No new grads.
• Arizona RN license is required
