Job summary

Del Corazon Hospice in Albuquerque, NM is seeking a compassionate Registered Nurse (RN) to perform hospice visits across Albuquerque and surrounding communities. This full-time role partners with the interdisciplinary team to provide skilled nursing care, symptom management, and compassionate end-of-life support in patients' homes.

Ideal candidates will hold an active New Mexico RN license with at least 1 year of hospice or home health experience, strong assessment and communication skills, and

Qualifications

• Active RN license in New Mexico (required).
• At least 1 year of nursing experience (hospice/home health strongly preferred).
• Strong assessment, communication, and independent decision-making skills.
• Ability to work autonomously in the field with excellent time management.
• Valid driver’s license and reliable vehicle.
• CPR certification preferred.

Responsibilities

• Case manage hospice patients along with needed visits in Albuquerque and nearby areas.
• Availability to make emergency visits after hours M-TH for assigned patients.
• Conduct patient assessments and manage pain/symptom control.
• Administer medications and treatments per physician plan of care.
• Educate and support patients, caregivers, and families through the hospice journey.
• Collaborate with the hospice interdisciplinary team (IDT) including social work, chaplain, aides, and medical director.
• Complete timely and accurate documentation in the electronic medical record.
• Serve as an advocate for comfort-focused, patient-centered care.

Compensation: $90,000 to $105,000 based on relevant experience, clinical skills, independence, documentation quality, and overall fit for the role. Mileage reimbursement, paid on-call stipend, and additional after-hours visit pay provided.
