Job summary

Ohio’s Hospice in Newark, OH is seeking a compassionate Registered Nurse (RN) Care Manager to provide expert hospice and palliative care to patients and families in Licking County. This full-time role coordinates care with an interdisciplinary team and requires travel within the service area.

The ideal candidate will be a graduate of an accredited nursing program, hold an active Ohio RN license, and have CPR certification. CHPN is preferred and supported by the employer.

Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) license in the State of Ohio, in good standing.
• Two years of acute care nursing experience preferred.
• Hospice, palliative care, home health, or community nursing experience is a plus.
• CHPN certification preferred.
• CPR certification is required.
• Basic computer skills and ability to accurately document patient care.
• Valid driver’s license and safe driving record.
• Reliable transportation and ability to travel throughout the service area.

Responsibilities

• Coordinate and deliver high-quality hospice care using the nursing process from initial assessment through ongoing evaluation.
• Assess patients and develop individualized plans of care based on their clinical needs and goals.
• Provide skilled nursing care to hospice patients in homes and care facilities.
• Monitor changes in patient condition and adjust care plans as needed.
• Manage symptoms and support comfort, dignity, and quality of life.
• Educate and support patients, families, and caregivers throughout the hospice journey.
• Collaborate with physicians and the interdisciplinary care team to coordinate comprehensive patient care.
