Qualifications

• Must be licensed as a Registered Nurse or Licensed Practical Nurse in the state of operation.
• One year of patient care experience in hospice, palliative care, oncology, or home health preferred.
• Working knowledge of hospice program objectives, payor plans (Medicare/Medicaid/Private Insurance) preferred.
• Basic computer knowledge/skill.

Responsibilities

• Assist with coordination of patient care within the Hospice Program and IDT.
• Ensure referrals/admissions meet customer service standards.
• Assist with assignment and scheduling of patient care staff.
• Supervise reviews of medical records, staff visits, and daily activity reports for compliance.
• Arrange hospice and supportive services.
• Assist with recruitment and training of field staff.
• Ensure compliance with safety and disaster plans.
• Ensure staff are trained on occupational exposure.
• Handle staff, physician, patient, and inter-departmental issues with timely follow-up.
