Qualifications

• Licensed Nurse with three to five years of hospice experience required.
• Current RN license to practice nursing in the state of employment.
• Prior experience as a discharge planner or case manager preferred.

Responsibilities

• Receive and respond to referrals from healthcare professionals.
• Assess patient eligibility for hospice services.
• Educate patients and families on hospice care.
• Complete necessary documentation and coordinate intake process.
• Serve as a liaison between referral sources and care team.
