Job summary

Residential Home Health and Hospice in Downingtown, PA is seeking a dedicated Registered Nurse (RN) for hospice care in patients' homes. A $10,000 first-year success bonus rewards milestone achievement during the initial year.

You will deliver compassionate, patient-centered care, perform assessments, manage symptoms, and educate families through the end-of-life journey. Travel to patient homes is required, with support from an education team and leadership that fosters growth.

Qualifications

• Active RN license in the state you work
• Two years of RN experience; hospice experience preferred
• Valid driver’s license and ability to spend ~20% of day driving to patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed and cared for
• Think critically, solve problems in real time, and ensure thorough EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
