Job summary

Grace Healthcare Services is looking for a skilled nurse in Jersey City, New Jersey to coordinate patient and family care. The role requires an active nursing license and a minimum of two years experience, preferably in an acute-care hospital setting.

• Provides skilled nursing as determined by the plan of care.
• Educates patient/family and other primary caregivers, in appropriate care techniques.
• Monitors the recertification/reauthorization process.

Job Description

• Coordinate the patient/family care and services including those of the IDT. (interdisciplinary team)
• Appropriately utilizes the skills and resources of contract personnel such as physical therapy when appropriate.
• Develops and updates plan of care with the IDT and others involved in the patients care.
• Provides skilled nursing as determined by the plan of care.
• Educates patient/family and other primary caregivers, in appropriate care techniques.
• Serves as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers, and others.
• May participate in providing bereavement support to families and associated bereavement activities.
• Ensure and document appropriate use of medication, durable medical equipment and supplies
• Participates in team meeting.
• Provide appropriate, coordinated care plans.
• Monitors the recertification/reauthorization process
• Reports necessary information to the Team Leader and other members of the IDT for on-call shift report.
• Responds to on-call issues as clinically appropriate
• Performs on-site evaluations and supervision of Grace HHAs.
