Job summary

Hearth Hospice is hiring a Full Time Compassionate Resource Nurse (RN) to provide patient care in and around the Fort Oglethorpe area. This role collaborates with intake, referring physicians, and leadership to promote hospice care and ensure proper admissions and pre-assessments in patients' homes or facilities.

The position requires an RN license, 1 year of relevant experience, CPR, and a strong comfort with EMR systems. Schedule is Fri-Sun, 10am-10pm, serving Dade and surrounding counties.

Qualifications

• Bachelor's degree in nursing or healthcare preferred.
• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care.
• Hospice/home health experience preferred but not required.
• Must be proficient with computers and EMR systems.
• Maintain CPR certification.
• Valid driver's license and transportation.

Responsibilities

• Review clinical referrals and determine eligibility for hospice services.
• Provide pre-assessments, admissions, and information to patients in home or facility settings.
• Coordinate with Intake, referring physician, and Hospice Medical Director to ensure compliance and certifications.
• Apply nursing principles and provide care, education, and support to patients and families.
• Participate in On-Call duties as assigned.
